Issues in care may include observations not done, patient not being given pressure care, patient not bathed, records not written, Dr not contacted for whatever reason.

Issues in construction may include wrong measurements, reel of cable left out, wires wrongly connected, water or gas pipes not connected, safety gear not worn, wprkman sat reading newspaper.

Issues in education may include records not written, planning not done, student problems not attended to, mis-information given, health and safety issues neglected, teacher leaving students alone.

Issues in a shop may include snapping at  a customer, not charging for something, incorrect pricing, out of date stock

Isues in parliament may include lying, fraud, falsification of records, secrecy, bias.

What does this tell us? Isues may occur in all jobs. The underlying theme is neglect to adhere to procedures where there are any. For this there may be several reasons such as staffing and training. A common link may well be attitudes of society- this may dictate how somebody deals with the situation. Many appear apathetic and complacent. We need to accept responsibility for ourselves and eachother. Do not adhere to  the herd.
