We must not fly on the defensive- any of us -but must confront these issues and aim to improve any failings in all quarters. Yes there have been failings in higher bodies. Yes individual staff have failed. Yes managers are responsible. In care homes the manager in particular, is well aware of this due to his or her registration. Yes it is stressful work and we all need support. No- staff are not always at  a low level. Accept this. Journals need to publish more work from practitioners- not only positive items. From negative, we can make recommendations.
